Leaves of Absence

Federal (FMLA) and state (OFLA) law requires that employers provide eligible employees with job-protected time off of work for their own serious health condition, to care for an immediate family member with a serious health condition, or to bond with their newborn or adopted child.

When the need for a leave of absence is known in advance, such as Parental leave or a surgery, please submit the Request for Family Medical Leave form with 30 days notice, if possible. Supporting medical documentation is required.

Under District policy, when an unexpected illness/injury causes an absence of more than five days, employees must provide a medical note. A doctor’s release to return to work is required if the absence is due to the employee’s illness/injury.
