For Renewal Applications:
- Annual Financial Statement (including income and expenses from all events and fund-raisers)
- Budget Plan for Next Year
- Fund-raising Schedule
- Certificate of Insurance (recommended)
For New Applications:
A copy of the proposed constitution and by-laws, copy of IRS determination letter, or a certificate of non-profit status needs to be submitted.
