SEED Survey
General Information Provided by Oregon Department of Education
The Student Educational Equity Development (SEED) Survey gathers information about the educational experiences of students in Oregon. The content of the survey varies by grade level and measures student perceptions of their opportunities, self reliance, and sense of belonging. Individual results will not be reported.

- Elementary - Grades 3-5
- March 10, 2026 - May 29, 2026
- Middle School - Grades 6-8
- March 10, 2026 - May 29, 2026
- High School - Grades 9-11 (Grade 12 does not participate in SEED Survey)
- February 9, 2026 - May 29, 2026

Who takes the SEED Survey?
All students grades 3 - 11.
2025-26 Annual Notice and Declining Participation Form for the SEED Survey
Oregon law (House Bill 2656) permits parents or guardians to decline, either verbally or in writing, to have their student participate in the SEED Survey. To decline participation in the SEED Survey, parents or adult students may submit the SEED Survey Decline Participation Form directly to the school.
