Leaves of Absence

Federal (FMLA) and state (OFLA) law requires that employers provide eligible employees with job protected time off work for their own serious health condition, to care for an immediate family member with a serious health condition, or to bond with their newborn or adopted child.

When the need for a leave of absence is known in advance, such as parental leave or a surgery, please submit the Family Medical Leave form with 30 days notice, if possible. Supporting medical documentation is required.

Under District policy, when an unexpected illness/injury causes an absence of five days or more, employees must provide a medical note. A doctor's release to return to work is required if the absence is due to the employee's illness/injury.
