​District Insurance Requirements - Facility Use

For the protection of yourself, your organization and the Beaverton School District, building users may be required to provide, at their own expense, a comprehensive general liability insurance policy or special event policy naming the Beaverton School District as an additional insured. The certificate of insurance must indemnify and hold harmless the Beaverton School District from any and all claims, liabilities or damages as a result of the use of the facility.  It may be mailed, faxed (503)356-4563 or emailed: risk@beaverton.k12.or.us.

 

Named Insured: Name of individual or organization named in Facility Use Request

 

Insurance Coverage:

General Liability –  Occurrence based General Liability with limits not less than $1,000,000 Combined Single Limit Bodily Injury, Property Damage and Personal Injury any one occurrence and in the aggregate.

Auto Liability – State of Oregon mandated limits for personal auto insurance will be accepted, for facility use, in most circumstances.

 

Additional Insured:

The District shall be named an additional insured on liability and auto policies and shall be provided a copy of the additional insured endorsement.
