District Insurance Requirements (basic)
Please refer the the Terms and Conditions in your contract, with the District, for specific Insurance Requirements.
|In an effort to protect our students, staff, community and District Property, please provide a Certificate of Insurance as outlined below. It may be mailed, faxed (503) 356-4563 or emailed: firstname.lastname@example.org.|
Named Insured: Name of individual or organization named in contract.
General Liability – to include premises operations, independent Vendors, products/completed operations and blanket contractual: $1,000,000 Combined Single Limit Bodily Injury, Property Damage and Personal Injury any one occurrence and in the aggregate.
Auto Liability – including owned, non-owned and hired vehicles: $1,000,000 Combined Single Limit Bodily Injury and Property Damage any one occurrence and in the aggregate. State of Oregon mandated limits for personal auto insurance may be accepted in certain circumstances. Auto coverage may be waived if vendor has no vehicle on District property.
Worker's Compensation – as required by law
Employer's Liability – in the minimum of $500,000 when the Vendor has one or more employees performing services under the contract.
The District shall be named an additional insured on liability and auto policies and shall be provided a copy of the additional insured endorsement.
