Consent & Access

Your privacy and safety are a priority at the SBHC. In general, adolescents have a right to privacy regarding some health information. If there is a safety concern, we are required by law to report. SBHC staff work with youth of all ages to involve parents/guardians in their care. If you have any questions about the confidentiality of your care, please ask our knowledgeable staff.

When do I need a signed consent form?

  • Any individual 14 years or younger needs a consent form signed by a parent or legal guardian to receive physical health services.
  • Any individual 13 years and younger needs a consent form signed by a parent or legal guardian to receive mental health services.
  • Reproductive and sexual health information can be received at any age without consent of a parent or legal guardian.

Can the SBHC work with my doctor?
Yes, the SBHC staff can coordinate with your Primary Care Physician regarding chronic illness care or treatment and other medical concerns. Licensed staff can also refer you to a specialist when needed.

Student Satisfaction Survey

Students between the ages of 12 and 19 who visit the Beaverton SBHC may be asked to complete an anonymous Satisfaction Survey after their visit. The survey asks about the student’s satisfaction and experience at the SBHC, as well as some general questions about their physical and mental health status. Students can refuse to take the survey and this will not affect their ability to get care at the SBHC.
